Error during playback: cannot load m3u8, 404

Hello.
Our set-up is functional; streaming live video from Akamai, encoded with Hyperstream via a m3u8 and self-hosted JW (6.4.3359). http://themetonline.org/live
There are occasional errors during playback. We are not able to manually reproduce the issue.

We receive reports from multiple physical locations that during playback the stream will stop and the error message will appear: Cannot load M3U8: 404 not found.

Examples from onsite at our streaming location. Error was observed about every 5 minutes this past weekend:

With all the steps in the process to get the stream working, where does this error point to a problem at? The Player? The Host of the player? The stream provider? The stream encoder?

The 404 error is occurring less now, but still happening.

Is this something on the website/player end or something on Akamai/Hyperstream's end?

I'm using Wowza Media Server 3.5 and this url is dynamic.. How can I place the crossdomain file?

Thank you once again!

P.S: I also play VOD files using the JW Player in my website and this error doesn't appear in that playback.
