Ability to embed a form? Capture email/name/phone at end of video?

Hi. Is it possible, when the video ends, to have a really simple form pulled in from somewhere, embedded into the player, to optionally capture the name etc of the person who's watched it?

We do not have built-in functionality for this, but you could use our Javascript API’s onComplete() event handler to display such a form directly on your site. With a little CSS, you could even put the form directly over the player.
