i have request for google video ads 2 time but the reject my application with the following reason i have working web site goodcast.net but still ads does not approve can any one guide my how i can get approved video adsense for my website

Upon review, you are currently not eligible for AFV due to one or more of the following reasons:
Invalid Sample Page
Please submit a sample page that properly serves video ads using the example IMA tag provided in our Video ad source interactive guide. The sample page must contain a working ad implementation and your full website template (header, navigation, content, footer). Your current page doesn’t have these elements or isn’t properly running (i.e. HTTP errors, no access, no test ads).
Video Content Only
We only approve AFV accounts for video content publishers that follow AdSense content policies (i.e. no copyrighted materials, adult, extreme violence). If you would like to monetize other types of content such as games, text, or image, please check out our AdSense product suite.
AdSense Policy Issues
Please ensure that you meet all our AdSense program policies, including AdSense for video policies.
YouTube Content
To monetize your YouTube content, please check out the YouTube Partner Program.
